Incident Reporting in Medicines Information (IRMIS)

All MI practitioners can report errors and near misses identified within their MI service via the Incident Reporting in Medicines Information Scheme (IRMIS) database. These incidents can be related to the enquiry answering service or to any publications/pro-active work. The scheme is intended to complement, but not replace, existing recording systems.

MI Managers can view, and if necessary, amend the detail of their entries at any time. It is also possible to produce anonymised reports of the national records on the database.

The Quality and Risk Management Group regularly review the records on a quarterly basis and highlight particular issues as appropriate. The IRMIS reports generated can be used as learning tools.

The guidance notes  describe how to use the IRMIS database.
